JOb Purpose

The incumbent will be responsible to provide strategic and technical leadership for the clinical design and implementation of person-centered, integrated care models focused on drug-resistant tuberculosis (DR-TB), chronic respiratory illnesses, and other common comorbid conditions in Sindh. This role emphasizes clinical quality assurance, advanced case management, and evidence-based protocol implementation in alignment with international standards. The Clinical Manager will represent the program in technical working groups, support the adaptation and scale-up of novel DR-TB regimens, and strengthen systems for drug resistance surveillance and longitudinal care monitoring.

RESPONSIBILITIES

  • Lead the design and implementation of integrated treatment strategies with a primary clinical focus on drug-resistant TB (including MDR, pre-XDR, and XDR-TB), ensuring alignment with WHO and national guidelines.
  • Provide technical leadership on the case management of complex DR-TB cases, integrating person-centered care approaches with optimized treatment regimens, including WHO-approved newer and shorter regimens, in consultation with pulmonologists, infectious disease specialists, and DR-TB clinical care groups.
  • Oversee the clinical rollout and ensure fidelity to protocolized care pathways, including baseline assessments, adverse event monitoring, pharmacovigilance, and treatment adherence interventions across decentralized treatment sites.
  • Supervise a multidisciplinary clinical team to ensure timely diagnosis, linkage to care, and clinical follow-up, while developing context-specific SOPs for case detection, household contact screening, and management of comorbidities such as diabetes or chronic lung disease.
  • Develop and manage a Respiratory Disease Expert Panel (RDEP) comprising specialists and consultants to effectively monitor and manage complex conditions and multi-morbidity cases.
  • Participate in technical working groups and consortia, providing clinical insight on the implementation of new drug regimens, resistance surveillance, and operational research.
  • Strengthen systems for drug resistance monitoring, including collaboration with laboratory networks, contribution to surveillance data interpretation, and feedback loops into policy and practice.
  • Collaborate with M&E teams to ensure rigorous clinical data capture, enabling real-time treatment outcome monitoring, adverse event reporting, and trend analysis for adaptive program management.
  • Work closely with grants and program leads to prepare clinical progress reports, regulatory submissions, technical briefs, and scientific abstracts for dissemination to donors, government, and academic audiences.
  • Design and implement a model to build clinical capacity in Sindh for improved management of drug-sensitive and drug-resistant TB, as well as other chronic respiratory conditions.
  • Provide ongoing mentorship and capacity-building to field-based clinical staff, ensuring adherence to clinical governance principles, rational prescribing, and infection prevention and control (IPC) standards.
  • Coordinate with logistics and operations teams to ensure the timely supply of medications, diagnostics, and PPE, and to support the operational needs of remote clinical teams.

REQUIRED KNOWLEDGE, SKILLS & ABILITIES (KSA)

Required Qualification

  • MBBS with at least 3–5 years of clinical experience.
  • Master’s degree in Public Health, Health Systems, Global Health, or a closely related technical discipline (preferred)

Required Experience

  • Minimum 5+ years in a supervisory or clinical management role, leading multidisciplinary teams and overseeing complex clinical operations, preferably within non-profit, development, or public health settings.
  • Proven experience working in rural or resource-constrained environments, with demonstrated expertise in clinical quality assurance and strong engagement with public sector stakeholders.

Required Skills

  • Advanced knowledge of drug-resistant TB diagnosis, treatment regimens, and programmatic management.
  • Familiarity with national and global DR-TB treatment guidelines, clinical algorithms, and pharmacovigilance frameworks.
  • Strong clinical acumen in managing complicated, co-infected, or treatment-refractory TB patients.
  • Experience in case-based discussion forums, expert panels, or DR-TB technical working groups.
  • Demonstrated ability to integrate clinical practice with surveillance, M&E, and research functions.
  • Technical writing experience (protocols, SOPs, case summaries, manuscripts).
  • Effective leadership and capacity-building skills in low-resource and decentralized settings.
  • Strong engagement with public sector health stakeholders and experience working in partnership with NTPs.
  • Commitment to ethical, culturally sensitive, and patient-centered care models.
  • Fluency in Sindhi (strongly preferred); willingness to travel frequently across field sites (required).
